External-Beam Radiation Therapy Raises Rate Of Secondary Malignancies Relative To Radical Prostatectomy In Men With Prostate Cancer

ORLANDO, FL (UroToday.com) - This international group of investigators evaluated a large cohort of men treated with either radical prostatectomy (RP n=6196) or external beam radiation therapy (EBRT n=4137) between 1983 and 2004. Hormonal therapy was not used in either group.

Need for subsequent extirpative treatment for bladder, lung, or colorectal cancer was examined. The investigators controlled for age, baseline co-morbidities, and year of treatment. History of EBRT resulted in a 3-fold higher risk of undergoing cystectomy (p=0.04), 1.8-fold increase in risk of undergoing lobectomy or pneumonectomy (p=0.02), and a 1.7 fold higher risk of having to undergo colectomy (p=0.02).

The question whether newer EBRT methods such as intensity-modulated radiation therapy (IMRT) confer the same risks remains unclear. Nevertheless, this data should be certainly remembered when counseling young patients with localized prostate cancer.
