The Journal's editor, Dr Kamran Abbasi, comments: "This special issue of JRSM attempts to capture the themes that will govern the future direction of the NHS by examining progress to date. Some of the leading names in UK medicine - players who have shaped and continue to shape healthcare in the UK - pick over the bones of our health service, offering critique, solutions, and personal anguish."

In this issue...

- Health economists Alan Maynard and Karen Bloor hope the next sixty years of the NHS will see more evaluation of planning initiatives pre-implementation.

- Sir Donald Irvine, the first GP to become President of the General Medical Council, on medical professionalism.

- Martin Marshall, clinical director of the Health Foundation, argues that the two "tribes" of doctors and managers needn't always go to war over quality.

- Dr Richard Smith, former editor of the British Medical Journal, discusses his crisis of faith in the NHS.

- Professor Steve Field, Chairman of the Royal College of General Practitioners, looks at the GPs role within the NHS today and in the future.

- Sir Liam Donaldson, the Chief Medical Officer, argues that the avoidable risks of healthcare are still too high and that quality and safety need to be on an equal footing to money and productivity.

- Health Minister Lord Darzi looks at the challenges and the benefits of evidence based medicine.
