Pediatric Urology - Cryptorchidism And Infertility, Collagen-Matrix Injection For Vesico-Ureteral Reflux

ORLANDO, FL (UroToday.com) - (Reported from the Annual Meeting of the American Urological Association) Highlights of the AUA included cryptorchidism and infertility. Collagen-matrix injection for vesico-ureteral reflux, and a novel neurosurgical procedure for incontinence in spinal cord injured patients.

Kim, et.al., showed that a biopsy of the cyrptorchid testis at the time of orchidopexy could predict future fertility. If there is absence of Ad spermatogenesis and < 0.2 germinal cells/tubule there is a high chance of the patient being infertile as defined as a sperm count of < 20 million.

Collagen injection of the uretrovesical junction to treat vesico-ureteral reflux is effective in 74% of children initially but 56% will have recurrence of the reflux at one-year. Calcifications may occur at the injection site and can mimic Ureteral calculi.

Rosenbaum showed that B12 levels can be quite low in children with long-term urinary diversions. 42% of these children had low or low normal B12 levels 7 years after urinary diversion.

The most interesting abstract detailed re-routing the L5 sensory nerve that innervates the knee to the S2 spinal cord. In an animal model, this allowed reflex voiding of spinal cord injured animals by scratching the knee. This may be very useful for humans in the near future.
