UroToday.com - Dr. Sanda reported on the PROSTQA, which assesses the QOL outcomes for patients treated with RP, XRT and brachytherapy. Sexual, urinary, bowel, and vitality data were collected and analyzed. The study was not randomized and the HRQOL was assessed within groups. Sexual function dropped most for RP patients, but was better for nerve sparing patients. Urinary domains dropped the most for RP but recovered over time although not to baseline. Rectal domains were worst for XRT. Neoadjuvant ADT worsened outcomes for the radiotherapy groups.

He discussed the impact of age, and older age had worse sexual outcomes for all treatment groups. Yet 45% of men receiving radiation reported no orgasms prior to treatment. Only 20% of RP men had no or limited orgasms prior to treatment. There was much better recovery of this function in men under age 57 years compared to older men. Urinary irritation, obstruction and incontinence were not very different in post-treatment recovery for younger or older age patients. The exception was worse symptoms 2 years after brachytherapy. In summary, age impacted sexual function in the older patient following surgery and urinary function in the older patient after brachytherapy.
