Fourth International Consultation On Incontinence (ICI) - Education And Continence Promotion Committee Highlights

PARIS, FRANCE (UroToday.com) - Ms. Newman presented the committee's findings and began by recommending a name change from the previous consultation to "Continence Promotion, Education and Primary Prevention." The committee aimed to evaluate the areas of urinary and fecal incontinence, painful bladder syndrome and POP. With respect to continence promotion, the committee made Grade C-D recommendation for education and Grade B-C recommendation for primary prevention. Ms. Newman highlighted the paucity of peer-reviewed data in this field, although she did mention that the Continence Foundation of Australia has published outcome data with respect to their continence program.

Ms. Newman highlighted one of the issues with continence promotion and lack of physician interest. In a study of 43 women with UI or FI, the treating physician did not ask 2/3 of women specifically about their incontinence. In a larger study involving over 4,000 women, each with UI or FI, only 61% and 41% of women in the respective groups sought care for their condition. Similar data was presented for women with POP.

Physician education is an important component of patient education, and the committee noted significant improvements in knowledge and moderate practice changes with a "train the trainer" approach. There is a lack of data regarding the effects of physician education on patient satisfaction or outcomes.

The utility of continence nurse specialists was discussed. Recent studies show improvement in outcomes and potential cost-savings with this approach.

Ms. Newman concluded by discussing primary prevention of incontinence and several areas where this may be useful including post-prostatectomy incontinence, perinatal education and in the geriatric population.
