"Please find attached this year's nomination forms for the Limbless Association's Prosthetic and Orthotic Awards 2008. We have changed the format of the forms, which we hope will facilitate and simplify the nomination process. There is now a nomination form for each category and respondents are able to nominate one person / company / product for every category. You are also free to nominate only in the categories that are of interest to you, so please feel free to reply with nominations for as many, or as few, categories as you wish. Please feel free to print and/or photocopy as many copies of these forms as you require, for distribution to friends, relatives or other patients / users at your centre."

"This year we are pleased to announce that the annual prestigious Prosthetic & Orthotic Awards are being held on November 1st 2008 at the Crown Plaza Hotel in Chester, following straight on from the Annual ISPO Conference, which will conclude in the afternoon and our awards ceremony and dinner will follow on in the evening at about 8pm."

"We are writing to invite you to nominate some one who you feel is worthy of recognition whether they are an amputee or a healthcare professional. Or if you feel your prosthetic or orthotic product has helped or made a difference in your life, why not fill out a nomination form to highlight this? Please complete the nomination forms in full and return them to us by the deadline of 1 October 2008."

- Douglas Bader Award for Inspiration
- Services to Other Users
- The Sam Gallop Life Time Achievement Award
- User Group

Professional Awards

- Outstanding Service by an Individual Professional
- DSC of Achievement
- Prosthetic or Orthotic Product Innovation Award

All nominations should be sent via email to maria@limbless-association.org otherwise please send the Prosthetic and Orthotic Nomination Form to Maria Jones, Limbless Association, Queen Mary's Hospital, Roehampton Lane, London, SW15 5PN.

The Awards are presented by the Limbless Association and are supported by ISPO, BAPO, APPLG and the BHTA.
