UroToday.com - A study by Dr. Christopher Roth, et al., evaluated the occurrence of urinary tract infection in patients with ureteropelvic junction obstruction and megaureters with respect to the role prophylactic antibiotics play in their treatment.

Urinary tract infections in this cohort were defined as culture-documented, symptomatic infections. Only children with high grade hydronephrosis were included.

A total of 92 patients met the study criteria. The rate of UTI in all patients was 4.3%. Infection rate was noted according to sex, obstruction level, hydronephrosis grade, or circumcision status, and no statistically significant difference was noted.

The group found that patients with antenatal diagnosis of hydronephrosis who were not maintained on prophylactic antibiotics had a low occurrence of urinary tract infections. They felt that in this cohort, antibiotic prophylaxis was unlikely to benefit most children with Grade III or IV hydronephrosis that resulted from upper tract obstruction.
