This conceptual review comprehensively presents the logic for ordering diagnostic tests to help clinicians investigate patients with first episode schizophrenia.

Although first-episode schizophrenia has become the focus of much clinical attention, there is still no agreed standard for a thorough medical diagnostic assessment.

The authors argue that the most comprehensive diagnostic tests do not necessarily provide the best results, as indiscriminate screening can result in too many false-positives.

Local considerations and individual-specifi c attributes are most important considerations in selecting the most appropriate test to aid diagnosis.
