AARP's Public Policy Institute will release a new survey and report that expose the struggles chronically ill patients and their caregivers face throughout the health care system. The study will be released at a Solutions Forum in collaboration with the National Partnership for Women & Families that will convene two expert panels to discuss the report's findings and health reform options to address the needs of people with chronic conditions.

The report, "Chronic Care: A Call to Action for Health Reform," concludes that overcoming barriers to improving chronic care requires adopting a range of health reform initiatives, and it offers specific recommendations for policy designs. The report includes two comprehensive surveys of Americans with chronic conditions and caregivers who work with chronically ill loved ones.
