Almost three quarters of patients with extensive burns die of the consequences of a severe infection. In the current edition of Deutsches Arzteblatt International, Timo A. Spanholtz of the Cologne-Merheim Burn Center and his coauthors discuss the acute therapy and follow-up care of burn disease.
Optimal treatment of severely burned patients necessitates collaboration between primary care physicians, emergency physicians and specialist departments for plastic surgery. During first aid from the emergency physician, the patient is removed from the danger zone and is administered adequate fluid, and drugs, over several intravenous accesses. Additional first aid measures include cooling and sterile covering of the burned skin.
The Central Office for Burn Injuries in Hamburg then organizes the necessary transfer to a specialist department. There are about 100 beds in Germany for severely burned patients, including 10 in the Cologne-Merheim Burn Center of Witten/Herdecke University. The lives of these patients are at risk from pneumonia, pulmonary failure, sepsis, and acute respiratory distress syndrome (ARDS). Bacterial infection of the wound is a frequent complication.
After intensive medical treatment, the patient may be given further wound care, physiotherapy, ergotherapy, and the necessary psychiatric treatment in a special follow-up ward. The primary care physician providing follow-up care must be able to recognize scar contractures leading to functional restriction and may refer the patient to the Burn Center once again for a new operation.

posted by Tracey Krawczyk on 9 Aug 2010 at 9:21 pmMy name is Tracey Krawczyk and my mother died from complications from a lifelong goiter, that turned into a benign tumor, closed off throat, my mother choked and had a stroke.My mother barely survived this, but she did. We lived in a small town and my mother was never treated for the goiter or checked for thyroid disfunction ,until I went into the medical field. When I went to work as a Ophthalmic technician in the medical field, I had the luck to work with a specialist in ophthalmic reconstruction sx, autoimmune diseases and THYROID DISFUNCTION. I spoke with him about my mother, I was a late baby, so my mother had me at the age of 38. Hallucinations and breakdowns occurred by the time I was 4. This was in her forty's which would be about right for pyschiatric symptoms to become full blown. I showed my doctor my mothers high school annual, she obviously was severely under weight with an obvious case of Graves. It shocked me to see her eyes were identical to the worse case patients that we saw routinely. This meant she was ill as a teenager, and undiagnosed. I asked her friends and they said she was very outgoing but hyperactive and very nervous and thin. I immediately had her worked up for tx for the goiter and thyroid disfunction. They gave her the cocktail and put her on synthroid. She calmed down greatly. My mother stayed heavily medicated with psych drugs rather than addressing the thyroid, not her fault lack of knowledge. I will always believe that CHILDHOOD TRAUMA, SEVERLY BURNED AGE OF FOUR BY ACCIDENT CAUSED THYROID AND MENTAL DISORDER. SHE WAS PLAYING IN KITCHEN AND HER OLDER TEENAGE BROTHER DID NOT SEE HER, TRIPPED OVER HER WITH A HOT KETTLE OF BOILING HOT WATER FOR TEA. IMPROPER MEDICAL CARE AND IMPROPER DEBRIDING AT HOME, CAUSED LOSS OF MOST OF THE MUSCLE ON HER LEG. SHE HAD TO LEARN TO WALK ALL OVER AGAIN. I COULD PUT MY FIST IN HER LEG THE BURN WAS SO DEEP, IT EVEN HIT HER GENITAL AREA. MY DOCTOR TOLD ME MOST CHILDREN IN 1924 WOULD HAVE DIED FROM SHOCK, FROM SUCH A BRUTAL ACCIDENT AND PRIMITIVE BURN CARE. All MY MOTHERS CHILDHOOD FRIEND DENIED MENTAL ILLNESS SYMPTOMS AS A TEENAGE. My mother was a very unpredictable woman with wild mood swings.She hallucinated very often when i was a child with christian issues stating there were demons beating her, and she would hit me as well stating the demons were doing this. Finally after crying annd screaming she would exhaust herself and fall alsleep. I always knew something was wrong with my mother even at the age of 4 and 5. I hid from her when necessary but I always tried to protect her also.My father was very physically and verbally abusive to her which made everything worse. After her stroke She was found in time at home and was put in icu, feeding tube inserted. I was told my mother would never be able to swallow or eat again. My mother was in her 70's and when she woke up wrote she would rather die. I would not accept this and had physical therapy to work with her and she recovered enough to eat baby food or I would take normal food and would puree it for her. Appetite returned and she was able to stay off feeding tube. I was forced to put her in a nursing home, when she was stable because my father was deceased and my older sister refused to help and my brother suffered from renal failure age 20. i stayed by her side until her death the tumor finally closed her throat off. I AM SORRY I WAS NOT THERE TO HOLD HER HAND BEFORE SHE DIED, IT TORTURES ME THAT SHE WAS ALONE WHEN SHE DIED. TO SUM THIS LONG STORY UP, PLEASE HELP ME, I AM A CERTIFIED NUTRITIONIST AND PERSONAL TRAINER NOW. I AM NOT WORKING BECAUSE I WAS HIT BY A DRUNK DRIVER AND I AM HAVING CERVICAL SX SOON, SO I HAVE TIME TO ASK. MY NUTRITION PROGRAM MENTIONED CRETINISM COULD CAUSE ALL HER SYMPTOMS, I NEED CLOSURE, SHE WAS NEVER TREATED FOR THRIOD UNTIL SHE WAS APPROXIMATELY 58 YRS OLD. COULD IT BE POSSIBLE THAT HER INSANITY WAS A DIRECT RESULT FROM THE THYROID DIFUNCTION AND POSSIBLY CRETINISM AS AN ADULT, OR IS CRETINISM ONLY IN INFANTS. PLEASE HELP ME WITH MY PAIN AND SORROW FOR MY MOTHER. I AM 50 YRS OLD NOW AND I SUFFER FROM PTSD, I HAVE NIGHT TERRORS ABOUT HER, AND I DONT SLEEP WELL. I AM FULLY FUNCTIONAL, BUT I HIDE MY PAIN FOR MY MOTHER AND ALL THE UGLY COMMENTS THAT HAVE BEEN MADE ABOUR HER FROM MY FATHER AND SISTER WHO IS STILL ALIVE. SHE IS IN HER EARLY SIXTY'S. I WANT THE TRUTH FOR MY MOTHERS LIFE AND OTHERS WHO HAVE SUFFERED OR WILL SUFFER NEEDLESSLY BECAUSE OF MISDIAGNOSIS.
