A Rutgers College of Nursing faculty member has co-edited a book that examines the transition children with special health care needs and disabilities face when they move from pediatric care to adult health care.

"Promoting Health Care Transitions for Adolescents with Special Health Care Needs and Disabilities," co-edited by Wendy M. Nehring, associate dean for academic affairs at the College of Nursing at Rutgers, The State University of New Jersey, and Cecily L. Betz, director of nurse training and director of research, University of Southern California, University Center of Excellence in Developmental Disabilities, Children's Hospital Los Angeles, is a book that helps health professionals develop an effective health care transition plan to ensure effective, coordinated health care as adolescents reach adulthood.

"Each chapter presents the different facets to this complex milestone in life concentrating on the health issues," said Nehring, who is also the director of the graduate program and associate professor at Rutgers College of Nursing and a Clifton, N.J. resident. "While often uneventful for adolescents without special health care needs and disabilities, it is a period in life for these teens that needs to be planned for well in advance and often involves a number of formal and informal support people and agencies."

The book, published by Paul H. Brookes Publishing Co., is divided into three sections: health care transition planning best practices, health care transition plans and interagency services. The themes of self-determination and person-centered care are integrated within each chapter, along with clinical examples and appropriate vignettes.

Each chapter contains real life examples from individuals with health care needs and disabilities, family members and health professionals. The experts writing in this book provide their opinions and expertise on how to manage this transition successfully.
