Dorsal Double-Layer Dartos Flap For Preventing Fistulae Formation In The Snodgrass Technique

UroToday.com- A Snodgrass hypospadias repair and its variations have become the preferred methods for treatment of hypospadias in the past 10 years. Throughout the literature fistula is the most common complication of this technique. The aim of this study from Erzurum, Turkey was to investigate the importance of the single and double flap to prevent fistula formation in the Snodgrass hypospadias repair.
Tubularized incised plate urethroplasty using a single or double flap was done in 74 consecutive boys over a 4 year period. The median age was 6.6 years old and the range was 1-15 years of age. In the first 29 patients designated as group 1, a dorsal flap was rotated laterally for covering the neourethra and in the remaining 45 patients designated as group 2, the neourethra was covered with dorsal double dartos flaps.
The group found that in the patients of group one, 4 had fistula and 1 had a glanular dehiscence, they also found that there were no complications in group 2. From their study they conclude that a well vascularized dorsal double layer dartos flap is helpful in preventing fistula formation. They go on further to state that this type of flap should be a basic part of the Snodgrass hypospadias repair.
