UroToday.com- A large armamentarium exists for the surgical treatment of proximal hypospadias. The proximal defects, which include the proximal penile urethra, the penoscrotal area as well as scrotal and peritoneal are the most difficult to treat, the goal is to create as near normal as possible penis that is functional and cosmetically acceptable. This study by Nuhoglu evaluated their experience with proximal hypospadias repairs.
The group had 171 cases of proximal penile, penoscrotal and scrotal hypospadias; they ranged of age between 3 and 28. The group evaluated early and late postoperative complication rates and factors affecting success. This was a retrospective study.
The group found that 16 cases underwent a Duckett repair, 20 had an onlay flap, 28 a free tube, 56 a Belt-Fiqua and 51 a Thiersch procedure. Overall the success rate at an early postoperative period was 60.8%. After a secondary procedure, the overall success rate went to 81.2%. The group did not find any statistical significance between any of the procedures. They did note however, that a secondary operation has a higher complication rate. This study nicely shows that proximal hypospadias repair remains to be one of the most difficult reconstructive operations performed by pediatric urologists.
