Phase III study presented at American Psychiatric Association Meeting
Shire Pharmaceuticals Group plc (NASDAQ: SHPGY, LSE: SHP.L, TSE: SHQ CN) announced that adults with attention-deficit/hyperactivity disorder (ADHD) experienced significant ADHD symptom control when treated with once-daily ADDERALL XR (a mixed salts amphetamine product). The 18-month interim analysis of a two-year Phase III trial led by Harvard investigators was discussed today during a platform presentation at the American Psychiatric Association (APA) annual meeting in New York City.
"Adults with ADHD can benefit from treatment with ADDERALL XR over the long-term because it can improve their ability to maintain focus, concentrate, and pay attention for longer periods of time, which may enable them to achieve more in professional, academic and social settings," said lead investigator Joseph Biederman, M.D., professor of psychiatry at Harvard Medical School.
Up to 65 percent of children with ADHD may still exhibit symptoms into adulthood and an estimated 4.4 percent of the U.S. adult populations are affected by ADHD. If left untreated, ADHD may seriously and adversely affect patients as well as their families, friends and coworkers. The disorder's symptoms of inattention and impulsivity, if untreated, can result in poor performance in the workplace and may increase adults' risk for other problems, such as drug abuse, anti-social behavior, and poor self-esteem, all of which can further impair social and family relationships, Biederman explained.
After these adult patients completed a four-week placebo-controlled trial with ADDERALL XR, they entered into a two-year extension study. The 18-month interim analysis of this extension study revealed that they maintained a significant reduction in their ADHD symptoms when treated with ADDERALL XR. For many patients, this sustained reduction in ADHD symptoms was at doses equivalent to those used in the initial four-week study, which reinforced previous findings that patients do not build up a tolerance to the effects of ADDERALL XR, says Biederman. ADDERALL XR was found to be significantlyefficacious across all doses.
Investigators measured patients' symptoms using a validated ADHD Rating Scale (ADHD-RS), in which scores are considered severe, moderate or mild. The ADHD-RS assesses the 18 individual items related to ADHD that are listed within in the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition. Nine of these items assess inattentive symptoms and the other nine items assess hyperactive and impulsive symptoms. Sample rating categories include "avoids tasks that require sustained mental effort" and "talks excessively." Patients with more severe ADHD improved the most on ADDERALL XR treatment in the four-week placebo controlled study.
After completion of the initial 4-week study, during which patients had significant improvements in ADHD symptoms within the first week, patients could opt to enroll in the long-term, 24-month, open-label extension study. All patients restarted ADDERALL XR at 20 mg/d. Then patients and physicians determined the optimal dose of ADDERALL XR (either 20, 40, 60 mg/day) through weekly visits, assessments of the severity of ADHD symptoms via the ADHD-RS, and dosage adjustments during the first part of the extension study. This dose optimization resulted in a average 45 percent reduction in total symptoms of ADHD in patients who had received placebo during the short-term controlled study. Patients who continuously received ADDERALL XR in both studies also experienced a significant decreased in total average symptom score.
An evaluation of the ADHD-RS Inattention subscale revealed that patients that switched from placebo to ADDERALL XR had a significant improvement, 43 percent reduction on average, in their symptom score. Likewise, an average 47 percent reduction in the ADHD-RS Hyperactive/Impulsive subscale was also noted for the same group of patients. Patients who continuously received the drug in both trials or those patients with interrupted ADDERALL XR treatment also experienced further improvement on both subscales.
Once optimized to the most effective dose of ADDERALL XR, few patients required dose adjustments for the duration of the long-term study. At six months, 14 percent of patients received 20 mg/d, 43 percent received 40 mg/d and 44 percent received 60 mg/d. At 18 months, slightly more patients received 60 mg/d (47 percent), and slightly fewer were receiving 40 mg/d (37 percent).
ADDERALL XR exhibited a good tolerability profile during both trials at all doses. The majority of side effects (95.9 percent)occurred in the first month of the trial, diminished over time and were generally mild or moderate in nature. The most frequently reported drug-related adverse events were common for this class of medications, and included dry mouth, loss of appetite, insomnia and headache.
The average age of participants was 40 and all had a history of ADHD before age 7. Of the participants, 60 percent were men and about 90 percent were Caucasian. Many of the participants were not diagnosed until they were adults and reported that their symptoms had negatively impacted their lives prior to treatment, indicating a public health need for earlier diagnosis and treatment, Biederman noted.

ADDERALLXR may not be right for everyone. ADDERALLXR was generally well tolerated in clinical studies. The most common side effects were decreased appetite, stomachache, difficulty falling asleep, and emotional lability. Patients should speak with their doctor if they have a history of high blood pressure or any heart conditions, glaucoma, thyroid problems, emotional instability, mental illness, or a known allergy to this type of medication. There is a potential for worsening of motion or verbal tics and Tourette's syndrome. Abuse of amphetamines may lead to dependence. The man who conducted this study ""Long-Term Safety and Efficacy of Mixed Amphetamine Salts Extended-Release for Adult ADHD." has a bad history of not reporting the grants he has received from pharmaceutical companies. I take this stuff too but don't be so quick to believe everything you hear from a doctor. This country is based on deceit and greed. Educate yourself.
The downside of adderall
posted by Joe on 8 Mar 2012 at 11:48 pmAs a child/teenager I struggled to perform in school. After being prescribed Adderall XR for a short period of time the only serious negative side effect I've noticed is decreased libido (sex drive). As someone who had a relatively healthy libido prior to being put on medication, I must say that the change is significant. However the benefits far outweigh the detriments. Anyone who is struggling with ADD/ADHD symptoms should consider Adderall XR. The slow release aspect is perfect for me because my sleep is unaffected if I take my medication in the morning. For many people the decreased libido may be a deal breaker, but remember that the drug does wear off after about 12 hours, meaning that decreased libido is hardly a factor at night. My experience may differ from your own, and I'd be interested to hear some other thoughts on this issue.
False. Adderall XR is not safe

The drug disperses differently based on a variety of factors, what's in your stomach etc. It stabilizes mood better than Adderall IR, but it destroys your libido. Also, you are no longer in control of the drug, it administers itself, meaning you can't keep track of what's you and what's Adderall. And it never wears off. There's no timeout or breather, or time to reflect. It's a zombie pill with less potency than its IR equivalent. Not to mention nobody knows what those 'beads' are made out of, polymers, or composite plastics or what. I took the XR for a month, only because the IR was extremely difficult to find for several months in the Northeast U.S. due some 'contaminated batch' or, the D.E.A. wouldn't sign some paper in time, so zero pharmacies on Manhattan island had the drug for five months! At any case, the XR ruined my life quickly, and I went back on the IR the next month.

posted by Carole on 20 Jun 2011 at 4:47 pmI was glad to read the success story posted by Bill. My story also has a good outcome. I have a history of depression - both postpartum and otherwise - which have been successfully treated w/SSRIs in the past. Even in the absence of depression, I tended to have a pervasive barrier in effectiveness and lack of focus. It seemed that I spent a whole lot of effort to do what my peers did routinely. I was always looking for things and waited till the last minute on projects, bills, taxes, etc. I often heard, "You're so smart, why aren't you performing better?" In 2009 I took a self assessment ADHD quiz and sought treatment. I tried the non-stimulant Strattera, but had side effects. I tried vyvanse with a notable improvement over 1 year in my work performance and effectiveness as a parent. In combination w/meds, I also saw a counselor (2008-10) and took a seminar in effectiveness (2010). My insurance changed and I started taking Adderall XR, 10-20mg per day, in Jan 2011. I just completed my work performance review and was thrilled to discover that my performance has again improved substantially, since starting the adderall xr. I am very pleased with these results.
Of note, on my days off/weekends I tend not to take any meds. I also occasionally forget to take my 2nd dosage during the workdays (I normally take 10mg at 7am and 10mg around lunchtime).

posted by Jo on 29 Mar 2011 at 11:46 amOn being in recovery an having ADHD, I was sober 17 yrs when diagnosed and was very nervous, but when following directions, it has been so beneficial for concentration I am not sorry. My mind doesn't spin and it makes me still, there are quite a few in recovery like us. Just follow dirctions and use your resources.
ADHD Success Story
posted by Bill on 11 Feb 2011 at 10:35 amI have an adult son who has struggled with ADHD. He was an average student in High School, but exhibited many of the common symptoms associated with ADHD. He had no desire to attend college and worked in construction for $10/hr for 2 years. He had very little confidence that he could get his degree because of how difficult schoolwork was for him. He decided to give it a shot and enrolled in a small junior college. His GPA for his first semester was a 2.9. He was encouraged, but had a difficult time even in "easier" classes. We decided to meet with our doctor to discuss his attention problems. Nobody is more against the unnecessary use of drugs than I am, and when the doctor prescribed an ADHD treatment I was hesitant. On the medication my son raised his GPA to a 3.9 in just over a year. He transferred to one of the most prestigious universities in the country and is on his way to medical school. Most kids with ADHD are full of potential. I am obviously against the illegal and inappropriate use of drugs, but I am confident that the majority of doctors understand the risks and prescribe the medications to those who need it. Please don't rob your child's future because of rumors or uneducated ideas. Consult your physician and get the help you or your child needs.
Adderal not for kids
posted by Kalen on 14 Jan 2011 at 8:27 amAdderal although will help with work performance motivation and organization but I think it is wrong to assume a child has adhd because children are naturally energetic and a huge number of teens simply use it to get high or pass a test. Plus its not very hard for a child to say "I can't concentrate in school" and get prescribed some Adderal. It is also addictive and can cause other addictions to worsen or relapse.the only thing I find wrong with this drug is that to many kids are on it, selling it, and abusing it.
Helpful For Add Adult In Workplace
posted by Nan Martin on 14 Sep 2007 at 4:49 amI have just started Adderall XR after several months of Straterra treatment, hoping to decrease problems in prioritizing, completing projects, organizing and being an effective employee, as well as being able to manage my personal life. I appreciate statistics regarding longer term treatment and tolerance.
I am 4 years sober and am considered at risk because of the high abuse potential. I would like to know if there are other cases of alocoholics/addicts/bipolars etc, who have benefited from this therapy.
