ORLANDO, FL (UroToday.com) - Researchers from the University of Tennessee in Memphis in a retrospective case control study of 713 consecutive patients treated with SWL and 146 consecutive patients treated with ueteroscopy (URS) analyzed demographic, laterality of the stone burden and treatment modality, and presence of pre-operative and post-operative diabetes mellitus to determine any significant relationships.

Of the 713 SWL patients 137 had pre-existing diabetes mellitus. Of the remaining patients 9.9% developed de novo diabetes mellitus during an average follow-up of 45 months.

Of the 146 URS patients 24 had pre-existing diabetes mellitus. Of the remaining patients 7.3% developed de novo diabetes mellitus with an average follow-up of 39 months.

When comparing the de novo diabetes mellitus after SWL and URS, only the total number of treatments 1.7 SWL versus 2.0 URS treatments, (P<0.001) significantly predicated the onset of diabetes mellitus. Age, gender, ethnicity, and laterality of stone disease were not significant risk factors for de novo diabetes mellitus.

The fact that this study shows an increased number of URS treatments significantly predicting the onset of diabetes mellitus compared to SWL suggests further prospective studies are indicated to include more detailed history taking such as BMI and other risk factors.
