November 1, 2016...my mom was recently transferred to Smithfield Manor from the hospital and Dr. Diaz is the facility's physician. We are fortunate. Dr. Diaz recommended course of treatment was on target and appears to have been missed at the hospital. The large number of patients in this facility in no way impact the level of attention and care that Dr. Diaz gives my mom. Our family has confidence and trust in Dr. Diaz.
